Outpatient Services
Tri-County Human Services, Inc. welcomes anyone who needs outpatient substance abuse and mental health services in Polk, Hardee, and Highlands Counties. We provide a variety of high-quality counseling services for adults and minors (ages 4-17). These services are individualized to meet the needs of each person entering services. Parental participation in all aspects of treatment is encouraged for minors. Counseling for children ages 4-12 are offered at the Avon Park and Lakeland locations only. All services (except drug screens) are available via telehealth. The outpatient clinics provide a wide range of behavioral health services including:

MEDICATION ASSISTED TREATMENT (MAT)
Opioid dependence is a physical addiction to prescription painkillers and heroin that affects many people in the U.S. Unfortunately, people who struggle with opioid dependence may be reluctant to ask for help because of the stigma attached to the notion of “drug dependence.” People who are opioid dependent may continue using opioids despite experiencing harmful consequences. If you or someone you know may be dependent on opioids, you are not alone. In 2013, there were nearly 2.4 million reports of people that had abused or were dependent on opioids. Opioids can be prescription painkillers. For example, oxycodone, hydrocodone, and fentanyl—better known by the brand names as OxyContin ® , Vicodin ® and Percocet ® ,—are opioids. The street drug heroin is also an opioid.

IFCC
The Intensive Family Centered Care Program (IFCC) provides free in-home parenting and family counseling to families and children that have a relevant future risk and/or history of substance use. The program lasts for 10-12 weeks and is offered in-home or office. The IFCC program utilizes an evidence-based and internationally recognized parenting and family strengthening curriculum developed for high-risk and general population families called Strengthening Families. The Strengthening Families Program (SFP) has been found to significantly improve parenting skills and family relationships, reduce problem behaviors, delinquency and alcohol and drug abuse in children and is also found to improve social competencies and school performance. Parents and children participate in SFP, both separately and together.
The Intensive Family Centered Care Program’s (IFCC) goal is to help families find solutions to a full range of personal concerns. These can include; crisis Intervention/reduction, parenting issues/skills, marital & family problems, emotional concerns for each family member, relationship issues, alcohol & drug problems, life adjustment, and mental health care.

PSYCHIATRIC SERVICES
The goal of Tri-County Human Services, Inc. Psychiatric Services is to provide excellent outpatient centered psychiatric care in a timely and efficient fashion. Our practice philosophy is that our patients deserve a respectful, caring and competent relationship with all members of our staff.
Services Include:
- evaluation of the need for medication,
- evaluation of clinical effectiveness and side effects of medication,
- prescribing, dispensing, and administering of psychiatric medications, and
- medications education (including discussing risks, benefits and alternatives with the individual or other responsible persons).

INTEGRATED BEHAVIORAL HEALTH PROGRAM
Tri-County Human Services, Inc. is currently providing behavioral health services at four (4) different primary care settings as part of an integrated health model. Counseling is being provided at Central Florida Healthcare (Lakeland, Mulberry, and Winter Haven clinics) and Lake Wales Care Center. These services include:
- Assessment
- Case management
- Individual therapy
- Family therapy
- Psychiatric evaluation
- Medication monitoring
These behavioral health services may also be provided in a licensed Tri-County facility that meets the needs of the person served. Continuing care is coordinated with the primary care physician.

JASA
The In-Jail Substance Abuse Treatment Program, (JASA), is an award-winning program in cooperation with the Polk, Highlands and Hardee County Sheriff’s Departments. The target population for the program is individuals with admitted substance abuse problems and co-occurring disorders. The program works towards identifying behaviors which cause significant problems in the participant’s life and then working towards helping the client develop a firm foundation for recovery.

PREVENTION SERVICES
Tri-County Human Services, Inc. provides school-based child and adolescent substance abuse education, prevention, and intervention services in Polk, Highlands, and Hardee Counties. The focus is on identifying risk factors displayed by youth within special school populations. These students may receive assessment and referral services as well as substance abuse education and group counseling. A wide variety of services are provided to students in grades K-12.

FAMILY INTERVENTION SERVICES (FIS)
The FIS team works collaboratively with the Department of Children and Families to provide assessment, linkage, and case management services to those families in the child welfare system. Our FIS staff provide these services in-home, in-office, or via telehealth.

PATH PROGRAM (Program to Aid Transition from Homelessness)
Tri County Human Services PATH program helps identify homeless individuals who are effected by Mental Health, Substance abuse and Co-occurring Disorders within the three county service area encompassing Polk, Hardee, and Highlands Counties. Our PATH staff are in contact transitional shelters, drop-in centers, jails, VA Service Centers, and food resource kitchens. At these locations, homeless individuals and families receive individualized services based on need.
PATH Outreach Services includes assisting the homeless population in order to improve their chances at gaining independent living, employment, medical/medication assistance, housing needs, or any other needed assistance.
PATH funded services are provided to homeless individuals on a case-by-case basis as funding allows and is appropriate for those with a mental illness and/or co-occurring disorder.

Helping Hands
a Thomas McMicken Legacy Program for jail transition
Healthcare: Access, Navigation, Delivery and Support
Helping HANDS is an award winning innovative jail transition program unique to Polk County. It focuses on Healthcare Access, Navigation, Delivery and Support for Polk County inmates returning to the community from jail. The program focuses on inmates who have had two or more arrests in the past 12 months and are receiving psychotropic medications while in jail. Referrals to the program come from the jail or via the community partner agencies listed below. Participation is voluntary, not court ordered.

- Winter Haven Hospital Center for Behavioral Health

- Peace River Center
- Community Paramedics (CPs) from Polk County Fire and Rescue engage inmates prior to their release from jail and up to six weeks after release to ensure the participant connects with a local behavioral health provider and accesses appropriate social services programs. The goal is to have the CPs meet participants in their homes within 48 hours of release to deliver a 30-day supply of all medications and provide an overall health review. CPs will be available to respond to the urgent needs of the participants.
- Certified Recovery Peer Specialists help participants troubleshoot problems that may arise and encourage them to be accountable to their treatment plans. They will assist with ongoing social service connections, advocate for the participants needs and mentor individuals to encourage success.
- ROOTS is a separate program within Helping HANDS which targets Helping HANDS participants who are homeless or at risk of homelessness and/or who need assistance identifying and applying for benefits such as SSI/SSDI. Helping HANDS clients who are identified with these needs are referred by the CP or Peer Specialist to ROOTS for further assistance.
