ASSESSMENT AND REFERRAL
Referrals are accepted for adult/adolescent and Behavioral/Medical Health Services on both a voluntary and involuntary basis. Services are provided without regard to race, religion, national origin, sexual orientation, disability/illness, or socio-economic status. All clients receive a thorough evaluation prior to or at the time of admission. Information gathered during this evaluation is utilized to make treatment recommendations and to develop an individualized treatment plan. Treatment services are provided in the least restrictive setting possible. Those clients in need of services not provided by TCHS are referred to an appropriate resource for these services.

ADULT OUTPATIENT SERVICES
Tri-County Human Services, Inc. provides adult outpatient Substance Abuse (SA) and Mental Health (MH) services at locations in Lakeland, Winter Haven, Avon Park and Wauchula.  The outpatient clinics provide a wide range of behavioral health services including assessment, medical services (Lakeland & Avon Park), case management, Targeted Case Management (Avon Park), individual counseling, group counseling, couples counseling, education groups, family counseling, aftercare services, drug screening by urinalysis, and referral for adjunctive and aftercare services as clinically indicated.  Intensive Outpatient (IOP) treatment is available at Lakeland location for women.  Length of stays in outpatient services range from three to nine months.

Outpatient services offer individuals the opportunity and means to overcome their substance abuse problems without regard for the client’s ability to pay for services.  Treatment success or failure is significantly correlated with the individual's motivation to change and with their willingness to follow treatment recommendations to develop a healthier lifestyle.

PSYCHIATRIC SERVICES
The goal of Tri-County Human Services, Inc. Psychiatric Services is to provide excellent outpatient centered psychiatric care in a timely and efficient fashion. Our practice philosophy is that our patients deserve a respectful, caring and competent relationship with all members of our staff. Services Include; evaluation of the need for medication; evaluation of clinical effectiveness and side effects of medication; prescribing, dispensing, and administering of psychiatric medications; medications education (including discussing risks, benefits and alternatives with the individual or other responsible persons); planning related to service delivery; and evaluating the status of the individual’s community functioning.

INTEGRATED BEHAVIORAL/MEDICAL HEALTH
Tri-County Human Services, Inc. is currently providing services at two (2) locations with Central Florida Health Care (CFHC) a Primary Care Provider, which include locations in Winter Haven and Lakeland.  Services generally provided at the site of a Qualified Primary Care Provider may include Outreach, Crisis Support Services, Assessments, Case Management, Individual Therapy, Psychiatric Evaluations/Monitoring/Medication Services and Urinalysis.  These continuing behavioral health services may also be provided in a licensed Tri-County facility that meets the needs of the person served.  Continuing care is coordinated with the primary care physician.

Integrating primary care services into behavioral health treatment settings may better prevent and address the physical co-morbidities that often accompany severe mental illnesses and addictive disorders. Other potential benefits of creating integrated systems in which primary care, mental health, and substance abuse services are meaningfully linked to treat the “whole” person may include:  the improvement of a patient’s retention in treatment programs & outcomes, improvement in overall health and functioning of recovering persons as a result of complex medical case management of substance-related and mental health problems, reduced utilization, and lower cost.

JASA
The In-Jail Substance Abuse Treatment Program, (JASA), operates in three counties; the Polk County Jail System in Bartow, FL, the Highlands County Jail in Sebring, FL and the Hardee County Jail in Wauchula, FL.  Tri-County Human Services, Inc., manages the Award Winning JASA program in cooperation with the Polk, Highlandsand Hardee County Sheriff's Departments. The target population for the program is individuals with admitted substance abuse problems and co-occurring disorders.  The program works towards identifying behaviors which cause significant problems in the participant’s life and then working towards helping the client develop a firm foundation for recovery.  Most of the participants in the program are of low income, have not completed high school and reside in areas of Polk, Highlands and Hardee Counties.  JASA provides treatment to the inmates in the jail system and is highly cost effective. The JASA program has saved taxpayer funds and freed space in the existing jails, limiting the need for new facilities. Participants learn new skills while in treatment that improve their lives and the lives of their families.

NEW BEGINNING TRANSITIONAL AND TREATMENT CENTER
New Beginning (located in Bartow, FL) is a residential transition treatment center which provides men with the necessary tools and life skills to re-enter their communities and maintain a lifestyle free from drugs and alcohol.  The center also addresses any untreated mental health issues.  The program is targeted to last 8-10 months.  
The men will work through a 4-phase program guided by the therapeutic community model.  This model includes, but is not limited to, the following array of services:

PREVENTION SERVICES
Tri-County Human Services, Inc. provides school-based child and adolescent substance abuse education, prevention, and intervention services in Polk, Highlands, and Hardee Counties.  The focus is on identifying risk factors displayed by youth within special school populations.  These students may receive assessment and referral services as well as substance abuse education and group counseling.  A wide variety of services are provided to students in grades K-12.  Substance abuse education is offered to "at-risk" students and also to the general student population.  Substance abuse prevention and intervention groups are offered at various sites.  As need and in cooperation with local school systems students are referred for adjunctive services as indicated.

PATH PROGRAM (Program to Aid Transition from Homelessness)
Tri County Human Services Path programs help identify homeless individuals who are effected by Mental Health, Substance abuse and Co-occurring Disorders within the three county service area encompassing Polk, Hardee, and Highlands Counties.  Tri-County Human Services, Inc. Outreach Specialist are in contact transitional shelters, drop-in centers, jails, VA Service Centers, and food resource kitchens.  At these locations homeless individuals and families receive individualized service based on need.

Outreach Services operate within a support service continuum where the Tri-County Human Services, Inc. Outreach Specialist identifies, screens, and assesses community service needs of the individual in need of or requesting services.  Tri County Human Services’ network of mental health and substance abuse services are used as an aide for assisting the homeless population in order to improve their chances at gaining independent living, independent employment, medical assistance, medications assistance, housing needs, or any other needed assistance.  The Tri-County Human Services, Inc. Outreach Specialist, in concert with Tri-County Human Services, Inc. Case Managers, will reach out to the local community. Tri-County Human Services, Inc. Clinics screen individuals and families eligibility to gain referrals and links to services, as available from other traditional and non-traditional service providers.

PATH funded services will be provided to homeless individuals on a case-by-case basis as funding allows and is appropriate for those with a mental illness and/or co-occurring disorder.

SELF-HELP GROUPS
Tri-County Human Services, Inc., acknowledges the importance of self-help groups in the treatment and continuing care programs of individuals with a substance abuse/addiction disorder. Consistent with this understanding, our treatment teams encourage Persons Served to participate in 12-step programs such as Alcoholics Anonymous, Narcotics Anonymous, and Celebrate Recovery. Many individuals receive an introduction to 12-step groups through their involvement in a TCHS Programs.

RESIDENTIAL SERVICES

MEDICALLY SUPERVISED DETOXIFICATION
Tri-County Human Services provides medically supervised alcohol/drug detoxification to adults residing in Hardee, Highlands and Polk Counties. Located in Bartow, Florida, the facility also provides substance abuse evaluation and referral services. Length of stays vary from 3-6 days depending upon the detoxification protocol utilized. Admissions to the program are accepted 24-hours a day.

The Detoxification Unit accepts referrals from a variety of sources including private citizens, public and private mental health/substance abuse treatment providers and the judicial system.  Once clients have completed detoxification and assessment at the unit, they are referred to public or private substance abuse and mental health treatment facilities as clinically indicated.  Referrals are also made to self-help groups such as Narcotics Anonymous (NA) and Alcoholics Anonymous (AA). Residential Level III Detox:  Residential Level III is a short term transitional program to help move an individual back into the community and/or other treatment program.  It is also available for those who have completed detoxification yet lack stable housing as they await formal admission to the Florida Center for Addictions and Dual Disorders or RASUW Center for Women.  Rather than have someone wait in an environment that would not deter a lapse in sobriety, the potential person served will be given an opportunity to participate in the level III program until their formal admission another program.

THE FLORIDA CENTER FOR ADDICTIONS AND DUAL DISORDERS
The Florida Center is a 55-bed residential facility for adults with co-occurring substance abuse and psychiatric disorders. Utilizing a skilled professional staff, the Florida Center provides up to 63 days of “state of the art” clinical intervention. To be successful, treatment of the substance abuse and mental disorders must occur simultaneously. Treatment includes the following services: substance abuse/mental health assessment, psychiatric evaluation, medication therapy, individual counseling, group counseling, HIV screening, and recreational therapy.  This program serves all 67 counties of the State of Florida. Residential Level II- 5-bed Program:  The 5-Bed Male Substance Abuse Program is for those individuals presenting with a primary substance abuse related disorder.  This program provides comprehensive assessment and treatment services.  Upon admission, individuals will be acclimated to Florida Center’s therapeutic community while beginning the assessment phase of their program.  Individual, group counseling and case management services are provided.  Because aftercare planning is so vital to recovery and integral to treatment success, Florida Center makes aftercare recommendations for all these individuals to transitional living facilities such as half way houses to facilitate continued sobriety and drug free living.  The 5-Bed Male Substance Abuse Program participants follow the same modalities as all persons served at Florida Center.

Residential Level III Florida Center:  Residential Level III is a short term transitional program to help the individual move back into the community and/or other treatment program.  It is also available for those who have completed detoxification yet lack stable housing as they await formal admission to the Florida Center for Addictions and Dual Disorders.  Rather than have someone wait in an environment that would not deter a lapse in sobriety, the potential person served will be given an opportunity to participate in a level III process until their formal admission is completed.  The level III program is also used as a step down to begin transition back to the community as appropriate

THE RASUW CENTER FOR WOMEN (Residential Assessment and Stabilization Unit for Women)
The RASUW Center for Women is located in Bartow, FL.  Adult women, (over the age of 18), residing in Polk, Highlands or Hardee County, Florida are eligible for admission to the center. The RASUW Center for Women is a fifteen (15) bed modified therapeutic community serving adult females with substance abuse and co-occurring difficulties.  The program provides priority placement and treatment for women who are pregnant or post-partum as well as women 18 years of age or older.  The primary purpose of this program continues to focus the needs of women who require substance abuse and co-occurring treatment.  The concurrent treatment of mental health, and substance dependence difficulties provides a safe residential environment, which is considered to be an integral aspect of the recovery process.  The RASUW Center for Women provides a ninety (90) day treatment regimen utilizing science-based techniques and strategies specifically designed to meet the needs of women.   Residential Level III RASUW Center for Women:  Residential Level III will be as short term transitional program to help the individual move back into the community and/or other treatment program. It is also available for those who have are awaiting formal admission to another treatment program.  Rather than have someone wait in an environment that would not deter a lapse in sobriety, the potential person served will be given an opportunity to participate in a level III process until their formal admission.

AGAPE HOUSE FOR WOMEN – HALFWAY HOUSE
The AGAPE House is a 10-bed, Residential Level III halfway house for women, with substance dependency disorders, who are 18 years and older. AGAPE House serves Polk, Hardee, and Highlands Counties.  The home is located Winter Haven, FL and provides its residents with easy access to community services, public transportation, and employment opportunities? The program provides a structured schedule of activities with a primary emphasis on rehabilitation.  Individual counseling, group counseling and case management services are provided as part of the daily schedule.  Agape House residents are required to be employed or involved in job training, education, or employment search activities so as to re-enter the community not only stabilized on drug or alcohol abuse, but as important, begin displaying her role as a productive and responsible community member.  Counselors and/or human services technicians staff the facility 24-hours per day.  Length of stay at the facility may be from three (3) to six (6) months.

TRANSITIONAL LIVING FACILITY
The Transitional Living Facility, located in Lakeland, Florida, provides transitional housing for women and their children. The focus of this unit is on reuniting mothers and children.  A case manager is available to advocate for residents and to assist them with identifying and accessing needed services. Transitional Living residents are required to be employed or involved in job training, educational, or employment search activities. Women may reside at the Transition House for 3-6 months.  Additional housing may also be available to individual and/or family throughout Polk County.

Tri-County Human Services, Inc. contracts with the Florida Department of Highway Safety and Motor Vehicles to operate programs in Polk, Highlands, and Hardee Counties.  These range from courses or programs for drivers that are securing their license for the first time to special programs for drivers with DUI related charges, other traffic infractions, license restrictions that include an Ignition Interlock Device (IID) and a program for those drivers with 5/10 year or permanent license revocations.  Let us help you get on the road again safely, and legally.
